Picked up a Picco 28 last week and haven't installed yet, what kind of glow plug should I use, med or hot? Do I have to use the OFNA plugs? What air filter/elbow works well with the 28?

A medium plug would be best. For the filter, use a 2 stage motor-saver style with any elbow that clears your spur gear set up.

Definetly a medium plug. Any 2 stage filter should clear with ease, but don't use the old style 1 stage filter.

The Motor Saver works with a bit of modification on the stock neck that fits over the flywheel. I used it successfully for quite while.
